Role Description: Service Transition Co-ordinator 

 

Title: Service Transition Co-ordinator 

 

Reports to: Service Portfolio, Transition & SRM Lead 

Location: Manchester / Hybrid 

Department: Technical Services 

Role Purpose 

The Service Transition Co-ordinator supports the successful transition of new or changed services into the live environment. This role ensures that service transition activities are planned, coordinated, and executed effectively, minimising risk and disruption to business operations. 

Key Responsibilities 

  • Transition Planning & Support 
  • Assist in the development and execution of service transition plans 
  • Coordinate resources and activities across projects to ensure smooth transitions 
  • Change & Release Management 
  • Support the implementation of changes and releases in line with governance and quality standards 
  • Ensure that all changes are documented, tested, and approved before deployment 
  • Service Readiness 
  • Validate that operational teams are prepared to support new or changed services. 
  • Ensure service documentation, training, and support models are in place 
  • Stakeholder Engagement 
  • Liaise with project managers, service owners, technical teams, and business stakeholders 
  • Communicate transition status, risks, and issues effectively 
  • Risk & Issue Management 
  • Identify and manage risks associated with service transition 
  • Escalate issues and propose mitigation strategies 
  • Continuous Improvement 
  • Contribute to the refinement of service transition processes and templates 
  • Capture lessons learned and feed into future transitions 

Skills & Experience 

  • Experience in IT service management or project coordination 
  • Understanding of ITIL framework, particularly Service Transition 
  • Strong organisational and communication skills 
  • Ability to manage multiple priorities and work under pressure 
  • Familiarity with service management tools (e.g. ServiceNow) 

Qualifications 

  • ITIL Foundation (preferred) 
  • Project management certification (e.g. PRINCE2, Agile) is a plus
